Medicaid Mental Health Targeted Case Management services provide case management to adults with a serious mental illness and children with a serious emotional disturbance to assist them in gaining access to needed medical, social, educational, and other services. Case management connects individuals with the appropriate community resources. It includes: advocacy, coordination of services, education, and follow up.
